
AHEC West is the coordinating partner for the Healthy Together program, a regional initiative funded by a $4.5 million grant from the Maryland Community Health Resource Commission (CHRC) Health Equity Resource Communities (HERC) grant. Healthy Together focuses on improving health outcomes for patients with heart disease and substance use disorder in the Georges’ Creek corridor and Garrett County.
Healthy Together involves a robust network of regional partners, including:
- The Allegany and Garrett County Health Departments
- The Allegany and Garrett County Departments of Social Services
- Mountain Laurel Medical Center
- Tri-State Community Health Center
Each partner has its own specific goals alongside shared objectives. These goals include increasing access to quality health care, providing transportation vouchers for medical appointments, improving food security, expanding access to primary care, and issuing security deposit vouchers to enhance housing stability.
