This website is provided as a public service by Maryland Area Health Education Center West (sometimes hereinafter referred to as “AHEC West”).
Of the information we learn about you from your visit to “http://www.ahecwest.org,” we may store the following: the domain name from which you accessed the Internet, the date and time, the Internet address of the website from which you linked directly to our site, the names of files or terms you searched for, and the browser used to access our site. This information is used to measure the number of visitors to the various sections of our site and identify system performance or problem areas. We also use this information to help us expand the coverage of the site to help users find health information. We do not use “cookies” on our website unless specifically stated.
If we request personal information from you, we are obligated under state law to provide you with the following information:
  • The purpose for which the personal information is collected
  • Any specific consequences to you for refusing to provide the personal information
  • Your right to inspect, amend, or correct personal records, if any
  • Whether the personal information is generally available for public inspection
  • Whether the personal information is made available or transferred to or shared with any entity other than the official custodian
We do not give, share, sell, or transfer any personal information collected on this site to a third party, unless otherwise specified.
You are not required to provide information to us. If we request information and you refuse to provide it to us, your refusal will not be held against you or prejudice our relationship with you.

When inquiries are e-mailed to us, we store the question and the e-mail address information so that we can respond electronically. Unless otherwise required by statute, we do not identify publicly who sends questions or comments to our website. We will not obtain information that will allow us to personally identify you when you visit our site, unless you choose to provide such information to us.
Information that you consider confidential should not be sent to us over the Internet.

AHEC West and your Personally Identifiable Information

THIS NOTICE DESCRIBES HOW YOUR PERSONALLY IDENTIFIABLE INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

AHEC West is committed to protecting any information that we collect that could be used to personally identify you to others. AHEC West is required by law to keep your Personally Identifiable Information (PII) protected from disclosures to others without your consent. As required by law, this notice provides you with information about your rights and our legal duties and privacy practices with respect to the privacy of PII. In order to provide assistance to you for your health insurance needs, AHEC West will ask for certain personally identifiable information and that information will be put into our contact management systems. This information will typically include your name, address, birthdate, phone numbers and email. AHEC West, its partner agencies and its Business Associates are required to follow the privacy practices described in this Notice, although AHEC West reserves the right to change our privacy practices and the terms of this Notice at any time. You may request a copy of the new Notice from AHEC West. It is also posted on our website at www.ahecwest.org.
Permitted Uses & Disclosures
AHEC West employees will only use your PII when doing their jobs. For uses beyond what AHEC West normally does, AHEC West must have your written authorization unless the law permits or requires it, and you may revoke such authorization with limited exceptions. The following are some examples of our possible uses and disclosures of your PII:
Uses and Disclosures of PII Required or Permitted by Law:
  • Information purposes: Unless you provide us with alternative instructions, AHEC West may send appointment reminders and other materials about the program to your home.
  • Required by law: AHEC West may disclose PII when a law requires us to do so.
  • Specific government functions: AHEC West may disclose PII of military personnel and veterans in certain situations, to correctional facilities in certain situations, to government benefit programs relating to eligibility and enrollment, and for national security reasons, such as protection of the President.
  • Lawsuits, disputes and claims: If you are involved in a lawsuit, a dispute, or a claim, AHEC West may disclose your PII in response to a court or administrative order, subpoena, discovery request, the investigation of a complaint filed on your behalf, or other lawful process.
  • Law enforcement: AHEC West may disclose your PII to a law enforcement official for purposes that are required by law or in response to a subpoena.
  • Other parties for conducting permitted activities: AHEC West may conduct the above-described activities ourselves, or we may use non-AHEC West entities (known as Business Associates) to perform those operations. In those instances where we disclose your PII to a third party acting on our behalf, we will protect your PII through an appropriate privacy agreement.
Your Rights
You Have a Right to:
  • Request restrictions: You have the right to request a restriction or limitation on the PII AHEC West uses or discloses about you.
  • Request confidential communication: You have the right to ask that AHEC West send you information at an alternative address or by alternative means. AHEC West must agree to your request as long as it is reasonably easy for us to do so.
  • Inspect and copy: You have a right to see your PII upon your written request. If you want copies of your PII, you may be charged a reasonable and cost-based fee for copying and postage. If AHEC West maintains your PII using electronic records, we will provide access in electronic format and transmit copies of the PII to an entity or person designated by you, provided that any such choice is clear, conspicuous, and specific.
  • Request amendment: You may request in writing that AHEC West correct or add to records. AHEC West will respond to your request within 60 days, with up to a 30-day extension, if needed. AHEC West may deny the request if AHEC West determines that the PII is: (1) correct and complete; (2) not created by us and/or not part of our records; (3) not permitted to be disclosed. If AHEC West approves the request for amendment, AHEC West will change the PII and inform you, and AHEC West will tell others that need to know about the change in the PII.

To Report a Problem about our Privacy Practices:
If you believe that your privacy rights have been violated, you may file a complaint.
  • You can file a complaint with AHEC WEST at 39 Baltimore St., Suite 201, Cumberland, MD 21502.
  • You can file a complaint with the Secretary of the U.S. Department of Health and Human Services, Office of Civil Rights. You may call the Department of Health and Mental Hygiene for the contact information.
AHEC West will take no retaliatory action against you if you make such complaints.
